Job summary
We are seeking someone to help shape and deliver the provision of co-ordinated care for vulnerable people in the London Borough of Hackney. The Neighbourhoods Meetings Development Manager will shape and lead the development of multi-agency team working across the health and social care system in City and Hackney within Neighbourhoods. The post holder will provide proactive support and have responsibility to ensure the management, development and improvement of Multi Agency Meetings within Neighbourhoods. We are seeking someone who already has or is prepared to develop their expertise to be a champion for person-centred, proactive integrated working that meets the needs of residents.
Main duties of the job
