Job overview
We currently have a substantive Band 6 Community Mental Health Nurse post within Coastal CMHT open to applications from Registered Mental Health Nurses. The team is based in East Ipswich and covers the area from East Ipswich through to rural areas of Felixstowe, Leiston and Aldeburgh.
You will be a qualified professional with experience of supporting people and their families, preferably in the community. You will demonstrate a strong understanding of your role as a Care Coordinator/Assessor and join our dedicated multidisciplinary team working closely with a range of partners to provide appropriate community interventions for those individuals referred to the team.
Main duties of the job
* Managing a caseload and the provision of mental health care services to people with mental health care needs.
* Provide planned care/risk assessment/screening for patients within their care plan and ongoing evaluation of patients’ needs and identify suitability for potential discharge in conjunction with the MDT.
* Initiate and attend patient-centred reviews, case conferences and other meetings as required.
* To advise the Responsible Clinician(s) and/or health and social care professionals of any change in the patients’ conditions and/or circumstances.
* To work within the MDT and to form working partnerships with other care providers.
* To ensure service users/carers are enabled and empowered to make informed decisions about their care and delivery of that care.
* Participate in the duty worker rota (Monday to Friday 09:00am to 17:00pm).
* Undertake the supervision of students that will be undertaking placements within the Team.

Detailed job description and main responsibilities
Within this role you will work with an established and supportive MDT. You will have knowledge, skills and experience of working across the range of complexity and severity in adult mental health.
You will be responsible for managing a caseload, assessing and planning treatment for your patients. Working with the patient to plan care and optimise treatment plans.

* Experience of working within a multi-disciplinary team
* Familiar with risk assessment and its management
* Previous community experience
Qualifications
* Nursing degree or equivalent
* Mentoring/Mentorship (ENB988 or equivalent)
* Evidence at social care/health related study
