Job Title
A Community Band 5 Staff Nurse, Rapid Response role has become available within our NHS AfC: Band 5 team.
The ideal candidate will be a passionate and dedicated NMC Registered Community Nurse who is enthusiastic about working in an integrated way with health and care providers to deliver high quality care to patients living in their own homes.
Our service operates 24 hours, 7 days a week, and we strive to care for acutely unwell patients at home, helping them to maintain independence.
We offer an extensive and supportive preceptorship programme, making this post suitable for both newly qualified nurses and those with years of experience.
Main Duties
1. Assess, plan, provide and evaluate evidence-based nursing care for patients experiencing a wide range of conditions.
2. Support patients and work collaboratively with multi-professional colleagues to help them avoid hospital admissions and facilitate timely discharge back home.
3. Provide IV antibiotics, line care, wound care, IM/SC injections, Peg Feeds, Venepuncture, Cannulation, Palliative care, and general nursing skills.
4. Work towards physical assessment qualification.
Person Specification
Qualifications
1. Degree in nursing or equivalent.
2. Registration with NMC.
3. Working towards Mentorship qualification (998-997).
Experience
1. Relevant experience/placements to the role including assessment and management of long term conditions, palliative care conditions and wound care.
2. Ability to manage time and caseload effectively.
3. Supporting service users, families and carers in dealing with people with nursing needs.
4. Working in a multidisciplinary team.
5. Experience in the delivery of healthcare.
6. Relevant placements including surgical, medical, palliative care.
