There is an exciting opportunity for a highly motivated and enthusiastic Healthcare Worker (fixed term part-time) to join an experienced multi-disciplinary team working in the community based at Laurel House near Canterbury city centre.
Under the guidance and supervision of the team, you will deliver an effective and competent level of care to adults with a Learning Disability who also have ADHD. You will be involved in assisting patients and their carers to attend appointments to be reviewed by a psychiatrist and advocating for them which may require liaising with other agencies, such as their social care teams and other community teams.
Main duties of the job
Support the Multi Disciplinary Team by:
* Offering appropriate support and advice to the patients in our care, and accurately record all care given as required on to RiO and validating own entries.
* Provide physical health care & physical health checks as required or instructed, documenting the physical health checks RIO.
* Supporting and carrying out patient therapeutic observation checks as required and instructed.
* Assist patients and the carers to attend appointments in a community clinic setting if requested.
* Work closely with the MDT and communicate information to the patient and carer under the instruction of the medical staff.
* Undertaking all relevant training as directed and participate in regular management/clinical supervision and appraisal.
* Seek advice and support if there are complex issues, for example issues relating to safeguarding.

Induction
