About us
We offer a range of services across Berkshire, with a focus on Reading, Wokingham and Newbury. The main base for our Clinical Nurse Specialist Team is at our Theale office near Reading, however we are open to applications from Nurses from across Berkshire and can discuss flexible working options.
At the Duchess of Kent Hospice in Reading and through our services in Newbury and Wokingham, we look after people living with life-limiting conditions and offer support for families too. We provide expert care and support across Berkshire.
About the Role
Sue Ryder are pleased to invite applications from experienced nurses to join a well-established friendly team as a Clinical Nurse Specialist in Reading. You will be supporting patients with an End of Life diagnosis to remain in their homes. This role is predominantly a telephone triage role.
The duties of this role will include:
1. Assessing referrals for admission to the service.
2. Performing comprehensive assessment of patients.
3. Planning, implementing and evaluating care delivery according to changing healthcare needs.
4. Identifying issues which are deemed to be actual/potential palliative care emergencies.
5. Compiling action plans to mitigate distress.
6. Ensuring such plans are developed, documented and communicated in a collaborative way with other professionals, patients and their families.
7. Using advanced assessment skills to identify reversible symptoms, develop a plan of care and implement.
8. Providing holistic specialist care for patients in accordance with their needs, wishes and culture.
9. Establishing and maintaining effective communication, often of a highly complex and contentious nature, with patients, carers/relatives, and professionals across health and social services.
About you
We are looking to recruit a registered nurse with experience as a community palliative care nurse or would consider a nurse with experience in palliative care and evidence of continued professional development in this specialty. You should have excellent communication skills and qualifications in advanced clinical assessment, mentorship and non-medical prescribing or be prepared to undertake training.
