Duties and responsibilities include, but are not limited to: Being responsible for the development of care pathways inclusive of pre-admission and proposed discharge; supporting the care team in the management of individuals with complex care needs. Making decisions to determine if patients require further intervention from the mental health teams (DCMH/PWS), including admissions to the place of safety or inpatient services.
