Alliance for Better Care are advertising on behalf of East Surrey Modality Medical Practice - Horley PCN, who have an exciting opportunity following the announcement of the new GP ARRS role. Healthy Horley PCN are looking for 2 newly qualified (within 2 years) GPs on a fixed-term salariedcontract until 31st March 2025 (with the view to extension). Healthy Horley PCN areseeking highly motivated and dedicated newly qualified GPs to join their multidisciplinaryteam under the NHS Additional Roles Reimbursement Scheme (ARRS). This flexibleposition requires a commitment of 4 sessions a week and includes an opportunityfor 1 of these sessions to develop an area of special interest in one of thefollowing:
1. Frailty / Anticipatory Care
2. Lifestyle Medicine
3. Urgent Care
4. Womens Health
Healthy Horley PCN are a high performing and collaborative network ofpractices with a strong commitment towards integrated partnership working,neighbourhood development and continuity of patient care. We use EMIS and ourmost recent CQC rating was Good.
The successful post-holder will be employed by East SurreyModality Medical Practice.
Main duties of the job
Healthy Horley PCN have an exciting opportunity to expand their Primary Care workforce by employing a newly qualified Salaried GP. The PCN deliver a wide range of services including a Children and Young Persons Wellbeing Service, Anticipatory Care / Frailty Hub, GP Integrated Mental Health Services, Employment Specialist Services and a Community Health Hub (Horley Health Hub). We are proactively engaged with the Neighbourhood Health and Wellbeing Networks and aim to remain at the forefront of community engagement and Integrated Neighbourhood Teams as they develop.

Horley Primary Care Network is an NHS Collaboration between 2GP practices East Surrey Modality Medical Practice and Wayside Surgery coveringHorley and surrounding areas (Outwood, Salford and Smallfield) and has 1 sitein Caterham (North Tandridge). The practices have a combined population of 44,000patients. There is a wide socio-economic and demographic served within thisarea which will provide an interesting and varied case mix. You will be joiningan enthusiastic team of clinicians and administrators including GPs, ParamedicPractitioners, Pharmacy Technicians, Clinical Pharmacists, PhysicianAssociates, Occupational Therapists, First Contact Physiotherapists, SocialPrescribers, Practice Nurses, Healthcare Assistants and more.

Primary duties and areas of responsibility
The successful post-holder will:
5. Provide a range of medical services as definedin the core GMS contract, additional and enhanced services where appropriate.
6. Assess the healthcare needs of patients withundifferentiated and undiagnosed problems and make professional, autonomousdecisions in relation to presenting problems.
7. Provide face-to-face and virtual consultationsfor routine and urgent care to meet identified patients need.
8. Conduct patient home visits (where required).
9. Manage patient care within the Practice,including managing patient queries, investigating and reviewing results,developing care plans and making necessary referrals in a timely manner.
10. Issue and sign computer generated acute andrepeat prescriptions (avoiding hand written prescriptions whenever possible),adhering to national and local guidelines.
11. Work with our clinical and support teams indelivering holistic and patient centred care.
12. Maximise the effective use of current clinicalcomputer systems.
13. Record clear and contemporaneous consultationnotes to an agreed standard using EMIS.
14. Ensure safe, evidence based and cost effectivecare in line with Practice disease management protocols and NICE guidelines.
15. Contribute to QOF & IIF requirements andrecording data appropriately.
Health Promotion and Patient Support:
16. Develop care plans for health in consultationwith patients and in line with current practice and disease managementprotocols, provide counselling and health education.
17. Screen patients for disease risk factors,lifestyle factors and early signs of illness and support patients in adoptinghealth promotion strategies and self-care principles.
18. Contribute to the delivery of anticipatory careplanning.

25. Work alongside the Practice Clinical Pharmacyteam to review and manage patient medications following the appropriatepolicies and local clinical guidelines.
26. Collaborate with health and social careproviders to deliver an integrated, safe service.
27. Actively participate in MDT and care conferencemeetings as required.
28. Attend team meetings and other clinical meetingsas necessary.
Risk Management and Compliance:
29. Adhere to all practice policies and procedures,including those related to prescribing, confidentiality, and data protection.
30. Work within current ICB policy for prescribing.
31. Apply infection control measures and manage risksin compliance with local and national guidelines.
32. Contribute to clinical governance throughclinical audits, data collection, peer reviews and significant event analysis.
Patient Safety and Confidentiality:
33. Ensure patient and carer confidentiality, inline with practice policies.

37. Newly qualified GP or recently completed GP training who obtained their Certification of Completion of Training (CCT) in General Practice in the last 2 years.
