Key Responsibilities: Patient Care: Assess, plan, implement, and evaluate individualized care plans for patients. Provide direct nursing care in patients homes or community settings. Ensure high standards of care based on current research and best practices. Clinical Duties: Manage a delegated caseload of patients under the supervision of a senior nurse. Perform initial and risk assessments of patients and provide immediate care as needed. Maintain accurate and confidential patient records. Team Collaboration: Work closely with other healthcare professionals, including GPs, social services, and voluntary agencies, to meet the needs of the local population. Provide clinical advice and support to other staff members. Participate in the training and supervision of junior staff and students Skills and Qualifications: Registered Nurse with a valid NMC (Nursing and Midwifery Council) registration. Strong clinical skills and the ability to work independently. Excellent communication and interpersonal skills. Basic IT skills for data collection and record-keeping.
