Job summary The position requires a qualified pharmacist with an independent prescribing qualification, who will operate within their professional limits as a member of a multidisciplinary team that includes GPs, ANPs, Physician Associates, Paramedics, and First Contact Physiotherapists. Main duties of the job The successful candidate will collaborate closely with the GP and other practice clinical and non clinical staff, playing a key role in supporting GPs and the broader primary care team to provide safe and effective care. They will provide medication-related patient services, including direct patient care. As a leader, the candidate will guide and support the practice team to ensure safe and effective prescribing, while maintaining and enhancing the quality of patient care within the practice. About us Hounslow Family Practice is a partnership of two GPs, providing care for 6,100 patients in Hounslow. Our exceptional clinical team includes 2 GP Partners, 2 Salaried GPs, a Clinical Pharmacist, 1 Practice Nurse, 1 Physician Associate, and 2 Health Care Assistants. Review the ongoing necessity of each medication, assess monitoring needs, and provide support to patients in managing their medications, ensuring they achieve the best outcomes from their treatments (i.e., medication optimization). Provide GPs with recommendations for medication improvements.
