London Borough of Bromley nare seeking : Care Manager Assistant: Social Care & Health Non-Qualified The Care Manager Assistant role is in the Penge & Beckenham Locality Team who work with Adults, Older People and Carers in providing personalised services which offers greater choice control over their care and support needs. MAIN PURPOSE: Under direction and guidance: Undertake assessments of Adults and Carers in line with legislation, policy and procedures. To provide practical support and information for Adults and Carers. Work with individuals to develop support plans by arranging and monitoring the delivery of services/payments to Adults and Carers. To work with your team colleagues and other professional staff to ensure that service user’s needs are met. Monitor, review services to ensure that they are provided effectively and in accordance with need. SKILLS & ABILITIES Able to demonstrate good assessment, verbal, and written communication. Skilled in working with others. Ability to listen and work with Adults and Carers in a strengths-based way. Able to cope with pressure. Ability to use information technology effectively. Skilled in valuing diversity and anti-discriminatory practice. Ability to work independently and flexibly. Ability to undertake home visits. EXPERIENCE Experience of having worked within Health and Social Care directly with Adults and Carers. Working across a range of agencies and understanding their role. QUALIFICATIONS A relevant social and health care qualification is preferable but not essential at BR8, i.e. NVQ. The progression criteria for BR9 requires successful completion of a relevant qualification e.g NVQ3 or able to demonstrate appropriate skills and experience at that level Minimum standard of English GCSE Grade A -C or equivalent Hybrid working will be considered: The LBB directive is currently to work 2 days in the office and the remaining days at home. Start Date : 17/02/2025 End Date : 30/05/2025 The salary is based on day work e.g bank hoilday rate shown. Hours : 36 per week
