Job Overview
The post holder is a pharmacy technician registered with the General Pharmaceutical Council. They are required to act within their professional boundaries, supporting and working alongside a multi-disciplinary team within general practice.
They will help to improve repeat prescribing processes in General Practice, reducing medicines waste, minimising clinical risk associated with the prescribing of medication by operating within clear protocols and guidance.
The post holder will work with a team of prescription clerks and clinical pharmacists within a prescriptions hub environment. They will be responsible for managing and assisting prescription clerks, and will work closely with pharmacists on an array of projects that ensure robust systems are in place to mitigate risk and facilitate safe prescribing.

Job Responsibilities:
Clinical:
* Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.
* Carry out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation.
* Support medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings.
* Support the Clinical Pharmacist in Structure Medication Reviews (SMR).
* Provide expertise to address both the public health and social care needs of patients.
* Manage shared care protocols and liaise with Clinical Pharmacists for complex patients.
* Support initiatives for antimicrobial stewardship.
* Assist in the delivery of medicines optimisation and management incentive schemes.
* Support the implementation of national prescribing policies and guidance.
Technical and Administrative:
* Support the practice multi-disciplinary team to ensure efficient medicines optimisation processes.
* Implement efficient ordering and return processes and reduce medication wastage.
* Provide training and support on the legal, safe, and secure handling of medicines.
* Develop relationships with other pharmacy professionals and members of the multi-disciplinary team.
* Support practice reception teams in streaming general prescription requests.
* Support the practice to deliver on QIPP agenda, QOF and locally commissioned enhanced services.

Key Working Relationships Internal:
* Clinical Lead for the multi-disciplinary team.
* GPs and General practice teams.
* Multi-disciplinary team members including Clinical Pharmacists, technicians, etc.

* The post-holder must comply with the organisation and Practice’s Health and Safety policies.

Person Specification:
* Professional registration with GPhC.
* BTEC/NVQ level 3 or equivalent in pharmaceutical sciences.
* Experience of working as a qualified pharmacy technician.
Knowledge and Understanding:
* Relevant advanced theoretical and practical knowledge of Primary Care Networks.
* Good clinical pharmacy knowledge.
Skills and Competencies:
* Computer literate with an ability to use required GP clinical systems.
* Excellent communication skills, verbal and written.
