Job Overview
We are seeking a dedicated and compassionate General Practitioner to join our healthcare team. You will need to manage a wide range of presenting health needs within a primary care setting. You will work alongside clinical colleagues ensuring the highest standards of care for all registered patients and providing emergency care as necessary.
The number of clinical sessions to be worked is to be agreed. Salary will be dependent upon experience.
Responsibilities
1. Clinical:
2. Receive patients with undifferentiated and undiagnosed problems and make assessment of their healthcare needs.
3. Make professionally autonomous decisions regarding presenting problems whether self-referred or referred internally within the Practice.
4. Screen patients for disease risk and early signs of illness.
5. Develop, with the patient, a care plan for health.
6. Provide professional support to members of the Primary Health Care Team.
7. Ensure that referral pathways are adhered to and actioned by use of Patient choice (Choose and book).
8. Admit and discharge patients to and from caseload and refer them to other care providers internally or externally as appropriate.
9. Actively participate in the Practices commitment to the Quality and Outcomes Framework (QoF) and the GP Specification, responding to specific system prompts relating to procedures required for specific patients.
10. Participate in the Practice on-call rota (currently between 8am - 6.30pm).
11. Provide counselling and health education.
12. Comply with relevant practice, PCT or department health policy/guidelines in relation to confidentiality and child protection.
13. Organisational:
14. Consult with patients in surgery and at home visits as set out in the job plan.
15. Make yourself available to answer patients' telephone queries or carry out emergency home visits or surgery consultations.
16. Attend promptly and diligently to pathology, radiology and other correspondence relating to patient care.
17. Record data in patient record systems to agreed standards within the Practice computer system.
18. Contribute to the evaluation/audit and clinical standard setting with colleagues at the Practice.
Requirements
1. Medical degree from an accredited institution and valid medical licence to practice as a General Practitioner.
2. Experience of using EMIS and other associated software - Microsoft Office.
3. Experience in medication administration and familiarity with medical imaging techniques is highly desirable.
4. Previous experience in home care settings is an advantage.
5. Excellent communication skills with the ability to build rapport with patients and their families, as well as with colleagues within the Practice.
6. Strong organisational skills with attention to detail in patient documentation.
7. Commitment to ongoing professional development and staying current with medical advancements.
8. Ability to work effectively both independently and as part of a multidisciplinary team.
