An exciting opportunity has arisen for an enthusiastic and motivated Breast Advanced Clinical Practitioner to Lead the Macmillan Breast Nurse Practitioner Team. We are seeking an experienced Advanced Clinical Practitioner in Breast Care with excellent communication and leadership skills.
You will lead and manage the Nurse practitioner team with service and personal development, while working alongside the Breast Consultant and Cancer Nurse Specialist teams to provide patients with a seamless service through diagnosis into treatment.
You will be supported in this role by our Lead Breast Consultant and Head of Nursing.
Ideally you will be trained to Msc level in Clinical Breast Examination and history taking with experience in symptomatic clinics essential to the role. You will also have experience in communicating benign biopsy results to patients.
Proven leadership, teaching and a portfolio of service development will be an advantage if applying for this role.
Experience in providing community teams with advice and guidance, provide long term follow-up of post treatment breast cancer patients and answering support calls for patients on supported self –management is desirable, but training is available to meet these requirements.
Thepostholder will be an integral memberoftheBreastCare team working collaboratively with nursing, midwifery and allied healthcare professionals undertaking advanced roles and responsibilities traditionally the remitofjunior medical staff under the directionofthe Consultant in chargeofthe patientscare.
The Advanced Clinical Practitioner (ACP) willworkwithin agreed protocols and guidelines exercising independent judgement in history taking, examination, assessment, diagnosis, and treatmentofa diverse and a caseload ofpatients.
The ACP will act as a professional resource for the multi professional teams, patients and carers. They will ensure that evidence based practice is inherent in all aspects of care and treatment and ensure this knowledge is disseminated throughout the clinical area, across the Trust and externally as appropriate.
The ACP will triage referrals either independently or with medical, therapy or other ACP colleagues and makeonwardreferrals in line withTrustpolicies.
A minimum of 70% of time must be spent in clinical practice

To accept clinical responsibility for undertaking the comprehensive assessmentofa caseload of patients, including those with a complex presentation, in line with evidencebasedpractice, legislation, policies and procedures. To accept referrals from and make referrals to consultants, GPs, and other healthcare professionals. Review the patient and act accordingly, thereby reducing length of time patients wait for intervention.
Improve the quality of the patient experience by identifying and meeting the individual clinical needs of patients. Act at all times in the patients best interests and as a patient advocate.
Provide specialist advice on patient care to the healthcare team and act as a specialist resource within the service and other areas within the Trust.
To support specialist nurses through working alongside them in the clinical area as required. To develop nurse led initiatives to allow service improvement and successionplanning.
To make recommendations for improvements in the provision and deliveryofcancer care across all the cancer and palliative care relatedspecialities
Produce an annual report to showcase key achievements including clinical activity, research and audit, patient feedback and education activity.
To participate in Macmillan reviews in order to drive service developmentforward.
Makes sure that people affected by cancer areawarethat they are interacting with a Macmillan Professional and know the full rangeofresources and services available throughMacmillan.
Actively engages with Macmillan Cancer Support to contribute expertise and experience and supports the Macmillanpriorities.
