This is an exciting time to join the Falkirk Health and Social Care Partnership as we are looking to appoint a Homefirst practitioner to our Hospital team. You will be a motivated worker who is enthusiastic and committed to improving the lives of our service users, practicing in a way that promotes better outcomes and focuses on strengths, rather than deficits. You will be primarily based at the acute hospital site at Forth Valley Royal. This is a full-time position, working 8am to 4pm, and is temporary for 6 months. Homefirst is a 7-day service, with workers working 5 days over 7, which includes a rota for weekend work.
Homefirst practitioners work in a high-paced, reactive and responsive environment as part of the Health and Social Care Partnership. This role includes partnership working to review and support discharge planning for service users. The team facilitates timely discharges to home or to intermediate care, working in partnership with the integrated health team to achieve the best outcomes for service users.
