Your Impact:
As a Field Supervisor, you will play a vital role in ensuring that our Home Care Aides (HCAs) deliver exceptional services to our clients, empowering them to maintain independence and quality of life in the comfort of their own homes.
Key Responsibilities:
1. Lead a team of Home Care Aides, providing guidance, support, and mentorship to ensure the delivery of compassionate and culturally sensitive care to our senior clients.
2. Conduct comprehensive welcome home visits for new clients, facilitating a smooth transition and establishing personalized care plans in collaboration with clients and their caregivers.
3. Implement innovative programs such as Healthy IDEAS to address the mental and emotional well-being of seniors, conducting assessments and providing case management to alleviate symptoms of depression and promote overall wellness.
4. Oversee the implementation of quality assurance measures, including regular assessments, evaluations, and surprise visits, to ensure compliance with established care plans and standards of service excellence.
5. Provide hands-on training and ongoing supervision to Home Care Aides, equipping them with the skills and knowledge needed to effectively meet the evolving needs of our clients and maintain the highest standards of care.
6. Serve as a liaison between clients, caregivers, and other healthcare professionals, advocating for the needs and preferences of our clients and facilitating communication to ensure coordinated and holistic care.
7. Conduct outreach activities to promote in-home services, engage with potential clients, and recruit qualified Home Care Aides, expanding our reach within the community and enhancing access to essential care services for seniors.
8. Collaborate with the In-Home Team Supervisors to facilitate discharge planning for clients transitioning from institutional settings, ensuring continuity of care and support during the transition process.
9. Maintain accurate documentation of client interactions, assessments, and service delivery in compliance with regulatory requirements and organizational policies, utilizing electronic systems such as Salesforce to track and record client data effectively.
10. Respond promptly to emergency calls or client matters outside of regular business hours, demonstrating a commitment to the safety and well-being of our clients and providing timely intervention and support as needed.
Requirements:
To thrive in this role, you should have:
1. High school diploma/GED and at least two years of relevant experience.
2. Proficiency in Chinese languages (Cantonese, Mandarin, and/or Toishanese) and English.
3. Demonstrate skills in supervisory activities.
4. Strong interpersonal skills and the ability to communicate effectively with clients and team members.
5. Detail-oriented with a commitment to thorough documentation and quality assurance.
6. Flexibility to work irregular hours, including evenings, weekends, and holidays, as needed.
Physical Demands:
1. Required to position and transfer clients weighing in excess of 100 pounds.
2. Required to frequently sit, stand, bend, and walk for extended periods.
3. Required to have yearly physical examination.
Environment and Scheduling:
Regular hours, Monday - Friday 9:00 am - 5:00 pm with the ability to work irregular hours as needed.
Work may be performed during irregular hours, Monday through Sunday (including holidays), from 8 AM to 8 PM in an INH client’s home or in the office.
Must be flexible and available to work after regular business hours 7 days a week to intervene in important client matters.
Utilize the team emergency cell phone, to effectively manage emergency situations outside of regular office hours on a rotational schedule.
