Hazelwood House is a 12 bedded and a 1 bedded bespoke pod, low secure unit for adults with a primary diagnosis of Learning disability and secondary mental health issues. We have a strong focus on recovery and enabling patients to reintegrate back into the community.
We are part of the Priory group and you will work closely with other clinicians in the region based at Lincoln, Mansfield & Nottingham.

Responsibilities
1. Lead a multidisciplinary team of professionals from nursing, clinical psychology, and occupational therapy.
2. Review treatment plans and coordinate discharge.
3. Liaise with Commissioners regarding admission, progress updates, and discharge planning.
4. Participate in a multidisciplinary approach to assessment, formulation, and management plans.
5. Provide evidence of GMC registration when requested by the Hospital Director or Chief Medical Officer.
Qualifications and Skills
1. Possess highly sophisticated communication and interpersonal skills.
2. Ability to work autonomously to agreed short and medium term objectives.
3. Good problem solver with systemic knowledge and experience.
4. Ability to translate best practice and evidence-based research into practical operational solutions.
Priory operates a non-residential medical on-call rota and the post holder is expected to participate on the on-call rota which is on a second on-call basis.
