Our Community Team for people with a learning disability and autism is committed to improving outcomes for the people we support. Key responsibilities...
You will hold a case load of people with learning disabilities and autism and will be required to fulfil statutory duties on behalf of the service. The post involves assessment, support planning and review, direct work with people who have a learning disability and or autism as well as general case management duties. You will also be provided with a laptop and a mobile phone to enable remote working.
