Westbury Group Practice is an innovative and forward thinking practice who have an exciting opportunity for an experienced Advanced Nurse Practitioner to join our Practice team as part of the Primary Care Network (PCN) Additional Roles Reimbursement Scheme (ARRS).
We are looking for a professional and dynamic ANP to join our team. The successful applicant will hold a Masters Degree, be qualified to Advance Clinical Practitioner level and ideally be an experienced Independent Prescriber. The post holder must be a good team player who enjoys working at the top of their clinical competency and who recognises the value of education, support and development within the workplace.
Main duties of the job
With a practice list exceeding 24k patients, WGP have developed and invested in a large multidisciplinary clinical team to support the delivery of high quality care to our patients of all ages with a wide variety of undifferentiated conditions.
The post holder will be a qualified Advanced Clinical Practitioner Nurse who, acting within their professional boundaries, will assess, diagnose, and manage patients with a range of acute, non-acute and chronic medical problems.
The post holder will provide both pre-arranged and same day visits to housebound patients with occasional visits to those living in care settings, demonstrating safe, effective clinical decision-making within the primary care role.
The post holder will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, whilst building effective working relationships with other external health care professionals.
Additionally, the post holder would be responsible for the management of the Practice infection, prevention and control (IPC) policies; additional training can be provided in this area if required.

Job responsibilities
* Undertake clinical nursing practice at an advanced level using knowledge and clinical skills to deliver holistic care to people accessing primary healthcare services.
* Triage, assess, diagnose and treat patients attending primary healthcare services with a range of acute, non-acute and chronic medical conditions.
* Directly admit patients to secondary care in acute medical need.
* Refer patients as appropriate to other members of the MDT, secondary care and other statutory and voluntary organisations.
* Referral to x-ray after having undertaken x-ray training as required by the designated hospital trust.
* Undertake and interpret a range of diagnostics tests where appropriate.
* Make clinical judgements appropriate to the expectations and demands of the role.
* Work and adhere to evidence based practice, recognised clinical guidelines and practice policies.
* Advise, support and mentor other members of the clinical team.
* Independent Non-Medical Prescriber. To prescribe safe, effective and appropriate medication as defined by current guidance.
* Effectively manage own time, workload and resources.
* Undertake home visits for acutely unwell housebound patients if required.
* Identify safeguarding concerns and make referrals as appropriate.
* Create, implement and manage IPC policies on behalf of the Practice.
Person Specification
Qualifications
* Degree or equivalent advanced clinical skills qualification
* Experienced Independent Prescriber
* RGN with current NMC registration
* Current UK driving licence and insurance
* Masters degree in Advanced Clinical Practice or working towards
Experience
* Minimum 5 years post registration
* Autonomous practitioner with at least 3 years recent primary and community nursing experience or in first presentation setting
* Nurse-led management of minor illness, minor ailments and injuries and chronic disease management
* Evidence of CPD in autonomous practice
* Ability to evaluate safety & effectiveness of own practice
* Involvement in mentoring/teaching learners
* Experience in leadership role & change management
* Experience telephone triage & visiting patient in own home/residential setting
* Good understanding of General Practice Sector
