Job Introduction
Are you looking for a role where you can make a real difference?
Our aim at Staffordshire is to create an environment where families are supported to stay together safely and live well in their communities by building on their strengths. This is the right thing to do.
We’re looking for passionate and motivated individuals to join our newly transformed district teams. We have a hybrid approach to work to assist your work-life balance, with a mixture of office and remote working.
You will be based in South Staffordshire for a fixed term of 12 months.
Main Responsibilities
You will:
* Support the Family Practitioner Leads to carry out the Local Authority’s SEND duties in relation to coordinating Education Health & Care (EHC) assessments.
* Produce EHC Plans and carry out annual reviews.
* Work closely with our children and young people with special educational needs and disabilities and their families as well as a range of qualified professionals in education, health, and social care.
* Develop collaborative and respectful relationships, promoting choice and independence and improving outcomes.
* Ensure that education, health, and care plans comply with legal, regulatory, ethical, and social requirements in addition to the policies, standards, and procedures.
The Ideal Candidate
It’s a fast-paced role so you’ll need to be able to manage your own caseload independently and as part of the team.
You will also:
* Need excellent written and verbal communication skills.
* Have knowledge of the SEND legislation. It’s important that you can reflect on practice and improve as a result.
Interviews will be held on Thursday 5th December 2024.
