More detail about the role Previous experience of working within the GP Practice or other health care provider would be desirable. You must be organised, self-motivated, forward thinking with excellent customer service skills. It is important that the candidate is empathetic towards patients using the Practice and can remain calm under pressure and deal with patient queries whilst answering calls and dealing with GP requests, be assertive and able to multi task and deal with a high volume of patients in reception whilst responding to other demands. Knowledge of medical terminology would also be an advantage. This role is a wide and varied one and the successful candidate will work closely with the patient and a multi-disciplinary team of clinicians to co-ordinate their healthcare needs, directing them to the appropriate service ensuring they get the most suitable care from whatever health or social care provider is appropriate. You will also be the first point of contact for the care homes covered by Tiverton PCN and you will build and maintain good working relationships with care home staff. You will also be allocated a caseload of identified patients and be required to ensure that their changing needs are addressed by taking into account local priorities, health inequalities and/or population health management risk stratification. Role Role Specific Responsibilities Generic Care Co-ordinator Responsibilities Care Homes Main point of contact at the Practice for Care Homes Check patients in care homes have up to date medication reviews and care plans and keep track of the numbers, Liaise with GP, Paramedic & Pharmacy Team Create and maintain a rolling programme of GP face to face reviews, at least annually for residents. New residents should be seen by a GP as a matter of priority on these visits. Make GP/Paramedics Aware of LTCs that are required Receive weekly ward round lists from the care homes and cascade to the relevant clinicians.
