INTERNAL MPFT APPLICANTS ONLY - DEVELOPMENT POST
The team is based at The Redwoods Centre in Shrewsbury and provides mental health assessment, treatment and support to people in Shropshire. As an integral team in the Urgent Care Pathway we work 24 hours a day, 7 days a week. We work with people who are experiencing a mental health crisis, require extra support alongside their regular support network or who can have the same treatment provided at home that they would receive in a Mental Health Hospital.
We are a team that consists of Band 5 and 6 Nurses, Band 3 Support Workers, Non-Medical Prescribers, Medical Staff, Occupational Therapists, Administrators and Business Support Staff. As we work with people with all types of mental health presentations, we need to have a range of skills and interests that create a strong and responsive team.
Main duties of the job
Mental Health Practitioner - Nurse, OT, Social Workers
1. Working in a multi-disciplinary team and in partnership with internal and external agencies, you will;
2. Provide specialist assessment and care planning in conjunction with service users and carers
3. Evaluate treatment plans and undertake home treatment
4. Triage and assess referrals received by the service in liaison with multi-disciplinary colleagues
5. Co-ordinate the work of the Team

Job responsibilities
This is a developmental role
The post holder is responsible for the assessment of care needs and contributing to; the formulation of the persons needs and the development of a relevant plan based upon the persons negotiated goals and relevant evidence based interventions. The post holder, in conjunction with the multi-disciplinary team, will provide interventions to support the person to meet their agreed goals and coordinate and/ or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway.
Upon successful completion of Band 6 competency framework the post holder will be progress to substantive Band 6. The competency level will be evaluated and evidenced through the supervision and PDC framework.
