If this sounds like you then come and join the Bed Management Team, a small, friendly team that works to ensure smooth andtimely transitions for our patients from our acute wards and out of Trust units into the community. Working Monday - Friday during core hours, working across the acute sites and will require you to be able to travel to visit our service users placed in hospitals across the region. Your role will include the management of the staff within the team working at band 4 level across the acute wards. There are two posts vacant. One of which will cover the Norfolk acute are and another covering the Suffolk acute area. For internal applicants we can offer fixed term roles as secondments with your current managers approval. Main duties of the job The successful applicant will provide proactive support to our Service Users throughout their admission in the identification of barriers to discharge and the solutions to overcome them and will liaise with NSFT colleagues, system partners and stakeholders daily. Your work will feed into the Patient Flow communication network on a daily basis. You will work autonomously to ensure continued momentum and progression through the care pathway for our patients whilst maintaining communication with your colleagues. Your work will be in conjunction with the Band 4 Discharge Facilitator staff on the wards to determine which patients present an indication of complex needs that will require further in-depth assessment to support appropriate discharge pathways. The postholder will also have regular contact with partnership agencies from Housing and Third sector agencies to develop networks to support the discharge process. The role will provide clinical management and navigation of mental health discharge pathways, whilst working collaboratively within a wider multi-organisational team alongside the Discharge Facilitators based on the acute wards. In this role you will develop your experience and knowledge of discharge pathways and out of hospital provision as well as influencing discharge decisions.
