W&HP PCN Care Coordinator - Ageing Well Team

Job Summary:
Woodsley and Holt Park PCN cover a large diverse population of approximately 110,000 patients in Leeds. We are looking to recruit a care co-ordinator to work in our new and developing Ageing Well Team. This provides an exciting opportunity to provide rewarding integrated care to this vulnerable cohort of patients.
Main Duties:
1. Develop the booking system and take responsibility for booking patient appointments.
2. Support clinicians to make referrals and signpost to specialist services quickly where necessary.
3. Monitor and record outcomes of all those accessing the service.
4. Gather and collate statistical and other information and data as required.
5. Conduct patient searches to identify population groups who would benefit from our service.
6. Support clinical staff in the identification and completion of QOF targets.
7. Identify and set up productive partnerships with local people and community groups.
8. Participate in internal and MDT meetings as required.
9. Support clinical visits where a second person is required.
10. Develop your role as the team becomes more established.
Communication:
1. Provide and receive information with empathy and understanding.
2. Take an active role in MDT processes providing administrative support.
3. Develop and maintain effective working relationships with other members of the primary care and wider team.
Administration:
1. Keep timely, accurate and complete electronic patient records.
2. Maintain appropriate documentation related to patients in their care.
Other Responsibilities:
1. Participate in discussions about service developments and improvements.
2. Participate in service audits and changes.
3. Contribute to the health and safety of others and the environment.
4. Participate in appraisal processes and in CPD/personal development plan.
5. Participate in mandatory training and in-service training.
Person Specification:
Experience:
* Previous experience working with the general public.
* Working autonomously and as part of a team.
Skills, Knowledge and Aptitudes:
* Excellent interpersonal, verbal and written communication skills.
* Excellent IT and computer skills.
* Time Management and ability to prioritise workload.
* Able to work independently and effectively within a team.
* Understanding of equal opportunity and diversity issues.
* Willingness to receive clinical training in the future.
Other Requirements:
* Current driving licence and access to own vehicle.
* Highly motivated and proactive.
Qualifications:
* English and Maths to GCSE Grade C or above or equivalent.
