The post holder will be responsible for delivering and leading highly specialist care within the Community Care Team. They will work closely with partner organisations in supporting and overseeing the assessment, planning, and delivery of personalised care, ensuring that patient-centred programmes of care are evaluated and managed within their sphere of competence and knowledge.
Main duties of the job
* Ongoing responsibility for the management of a defined caseload
* Accept referrals from health and social care professionals and voluntary organisations and self-referrals within a defined population
* Responsibility for the organisation, management, and development of the Community Care Team
* Assessment, implementation, evaluation, and reassessment of care for all patients referred to the team
* Plan programmes of care for patients with diverse and complex health needs
* Primarily visit patients in their own homes or other places of residence (e.g., residential care homes)
* Responsible for the delivery and organisation of clinics to meet the changing needs of the service (e.g., dressing and phlebotomy clinics)
