Job summary The post holder will be responsible for supporting individuals and families who have been diagnosed with a genetic or familial disorder (Lynch Syndrome, the polyposis syndromes or individuals with a high risk family history of bowel and/or related cancers), in order to minimise their risk of getting cancer. Working as part of a multidisciplinary team with medical and nursing consultant support, the nurse practitioner will have clinical responsibility for their own caseload and will act as a clinical expert; assessing, planning and delivering care to patients via face to face, video and telephone nurse-led clinics. As part of the role, the post holder will provide expert genetic counselling in bowel cancer genetics and will be a source of expert genetic information to health care professionals. Working closely with our colleagues in clinical genetics, the post holder will contribute to service development to ensure that genetic testing is appropriately mainstreamed into clinical care. The post holder will be expected to undertake research and clinical audit in this highly specialist area, ensuring the continuation of clinical excellence, underpinned by research based practice. Teaching our patients and families about their conditions and raising awareness of these conditions is also an essential part of the role. Main duties of the job Assist the lead nurse with the comprehensive and effective management of patients with inherited and familial gastointestinal cancer syndromes - polyposis, Lynch and family cancer Assess, plan and implement care, providing genetic counselling and organising predictive genetic blood tests as appropriate. Offering screening to newly diagnosed patients, monitoring follow-up procedures and providing specialist advice and support to the patients and their families. Specialising in the defined area of gastrointestinal inherited cancer syndromes, providing specialist education, training and support to the multidisciplinary team regionally, nationally and internationally. Maintaining the integrity of the specialist database which contains a register of Polyposis and Lynch Syndrome related cancers and other manifestations of the syndromes. Undertaking research and clinical audit in own specialist area, ensuring the maintenance of clinical excellence, underpinned by evidence-based practice.
