Working on a rota basis covering shifts 0600-22:00 including weekends and bank holidays. Shifts on weekends and bank holidays will be paid at enhanced hours.
Main duties of the job
The role of the Health Advisor is to ensure that Patients/Carers receive the best outcomes, which means providing the right care in the right place at the right time.
You will ensure that each patient/Carer receives a Making Every Contact Count (MECC) assessment. You will coordinate and collate all incoming MECC referrals and make the necessary bookings into the services where appropriate.
You will ensure you deliver a high-quality patient-centric experience within the Care Coordination Hub, supporting all other functions in delivering outstanding patient/Carer experience. To provide patient/Carer focused telephone and administration services that assist and direct patients/Carers in accessing the appropriate service or healthcare professionals.

Person Specification
Experience Essential
* Previous experience of working in an office environment and carrying out general office/administrative procedures
Desirable
* Previous experience of patient appointment booking systems e.g. SystmOne
Qualifications Essential
* NVQ level 3 in Business Administration/Customer Services or equivalent level of knowledge and experience
* Word processing to RSA 3 standard or able to demonstrate this level of skill
Knowledge Essential
* Comprehensive knowledge of a range of administration procedures and practices
* Knowledge of a range of software programmes including Microsoft Word, Excel, Outlook and EPR systems
Desirable
* Knowledge of SystmOne functions and other clinical reporting systems
* Knowledge of clinic/appointment processes
* Knowledge of Making Every Contact Count (MECC)
