Job summary We are looking for a paramedic who can think outside of the box as part of our established Community Virtual Ward and Urgent Care Teams to continue the provision of effective urgent care support to local residents in Sutton.Using your urgent care experience this is a great opportunity to work within our Multi-Disciplinary Team, supporting our local residents to stay in their own homes, avoiding hospital admission where possible.You will have clinical responsibility for assessment and treatment planning for adults referred into the service. Supervising Junior Colleagues, and working in collaboration with colleagues acrossSutton Health and Care and within the wider health and social care network, you will help to optimise patient journey, care, wellbeing and outcomes. Main duties of the job Act as an autonomous practitioner, responsible for the management of patients withthe support from the MDT/ VW Clinician Provide high quality, comprehensive holistic assessment (including physical assessment) and treatments for patients and their families referred to the service utilising a Single Assessment Process to achieve person centred care. Provide assessments of risk and complex situations as required, utilising knowledge and skills to interpret and make accurate analysis of patients' and carers' needs The post holder may be responsible for staff and in accordance with this - allocating duties and work, providing clinical support and leadership, prioritising workloaddemands and allocating resources as appropriate. To work closely with the MDT clinicians and provide support where required. This willinclude virtual monitoring, health and wellbeing checks and pulse oximetry Provide a rapid response service the community nursing teams meeting a 2 hourresponse time for urgent cases.
