Within this role, you will provide a person centred,statutory social work service for a caseload of patients in accordance withrelevant policies and procedures. You will be assessing and planning supportplans to meet individual needs as well as working with partners to safeguardservice users.

Conduct assessments to determine clients' needs and develop individual careplans
Provide counselling and support to individuals and families facing challenges
Liaise with external agencies and professionals to coordinate services forclients- Advocate for clients' rights and access to resources
Maintain accurate records of client interactions and progress

Qualifications
* HCPC registered
* A degree or equivalent in Social work
