Main Duties of the Job
The postholder must be an experienced Healthcare Assistant who can support in managing specific cohorts of patients with a range of acute and chronic medical conditions. They will work as part of the team, delivering care within their scope of practice to patients within the surgery and conducting home visits. They will deliver and assist clinical staff in the provision of treatment, preventative care, health promotion, and patient education. They will carry out baseline observations such as pulse oximetry, blood pressure, phlebotomy, BMI, and temperature as directed and record findings accurately. They will support with the management of chronic disease, delivering opportunistic health promotion where appropriate. The post holder may complete basic care plans.
This role is initially offered on a 12-month contractual basis, with further extensions to be discussed. A travel allowance will be paid for completing home visits.
About Us
The role will support four practices within the Glossop area, with approximately 33,350 patients. This role will work alongside PCN Additional Roles in practice including social prescribing link workers, mental health workers, paramedics, and pharmacists amongst others.
Job Responsibilities
Healthcare Assistants play an important role; they work closely with GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed.
Person Specification
Experience
* Good organisational skills
* Ability to work flexibly to meet the demands of the service
* Demonstrate an interest or previous experience in caring for patients in an acute setting
* Basic knowledge of IT packages
* Experience of working in a team
Qualifications
* Level 1 literacy and numeracy (GCSE English and Maths Grade D-G)
* NVQ level 3 in Healthcare
