Bentley Medical Practice are seeking a Advanced Nurse Practitioner to join their friendly, experienced practice team.
The hours can be part time / full time up to 37.5 hours per week, Monday - Friday between 8.00am - 6.00pm.
Six weeks annual leave pro-rata.
Gynaecology and Paediatric experience desirable.
Main duties of the job
The Nurse Practitioner will consult with patients who wish to be seen by a healthcare professional; diagnosing and treating patients presenting with minor illness and complex care management and assessment of patients with long term conditions.
About us
We are a friendly practice with four GP's, three ANP's, three Practice Nurses, two HCA's and a Phlebotomist along with an administration team.
We use SystmOne clinical system.
We have a Nurse Lead, Louise Johnson, who holds regular Nurse Meetings and we provide regular support and supervision monthly with one of our GP's.
We encourage professional development in areas of interest.
Job responsibilities
1. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
2. Provide long term conditions reviews and management in line with QOF to ensure effective patient care and management.

Person Specification
Qualifications
* To be a registered nurse with a NMC personal identification number.
* To hold an Independent Prescribing Certificate.
* To hold a Physical Assessment Skills Certificate.
* Experience of Primary Care.
* To hold a certificate in assessment of paediatrics.
* To hold a certificate in Long Term conditions management.
* To have experience of gynaecological conditions.
