The Discharge Social Worker will be expected to work in partnership with service users and carers, identifying and utilizing strengths and working closely with other agencies and voluntary groups to ensure good outcomes which, as far as possible, maintain independence.
The Discharge Social Worker will have a good working knowledge of The Care Act, Mental Health Act, and Mental Capacity Act/DOLS and other social care legislation.
