This job description provides a broad framework within which the post holder will undertake a range of duties. The successful applicant will: Act as link person for young people & their families/carers with other professionals & services Promote self-esteem & optimism whilst assisting in service development. Be aware and understand the guidelines relating to Child Protection issues. Access IT systems, enter and retrieve basic data and log out, effectively respond to telephone enquiries and escalating to transition co-ordinator or senior colleague as required Liaise with relevant colleagues, and work closely to maintain communication channels with key stakeholders by keeping up to date with national transition guidelines, policies, procedures, pathways and materials. When necessary, assist in the organisation and co-ordination meetings, set agendas and take minutes & monitor action-trackers Contribute to data collection as required Deal with highly sensitive information and patient data, adhering to IG regulations Maintain confidentiality regarding children and families at all time To promote a young peoples focused approach to care in collaboration with all relevant health, social care and other agency professionals To assist the organisation and take part in trust wide transition events Undertake learning activities with young people that are enjoyable, challenging & educational Promote and support the young person with sexual health advice Promote and support discussion with the young person on general health, diet, alcohol, drugs and physical wellbeing Promote and support the young person with self-care and medication management Promote discussion on education and career pathways Act as an advocate for families throughout the transition process Limit risk of cross-infection to patients and other staff by adhering to all infection control guidelines & advice Be aware of and adhere to the Trusts infection control / Risk management policies & attend regular updates on fire and manual handling in line with Trust policy Advise parents/carers on appropriate health and safety issues Maintain personal standards of health, security and safety Promote anti-discriminatory practice Maintain the confidentiality of information Promote and support individual rights and choice within service delivery Acknowledge individuals personal beliefs and identity Support individuals through effective communications Provide support to young people and their families in preparation of transition to adult services, and to signpost to appropriate follow on services Engage, motivate, support and encourage teams in development and application of transition pathways Work with Transition Co-ordinator to provide support for each individual referred to the service To assist in the organisation of MDTs or attend reviews e.g. EHCP Support specialities in using transition tools e.g. health passports, acute admission plans etc. Enable the family/carers of young people to empower the young people in their care Use expert professional judgement, analytical and problem solving skills (evidence based) in the surveillance and recognition of changes in patients condition, referring changes to medical clinicians as necessary. Demonstrate and utilise specialist skills and knowledge to provide innovative practice in transitional and general nursing care To promote a seamless service to children/young people through hospital and in the community as required, providing on-going and structured education and support for young people and their families To maintain a professional attitude and work in accordance with the NMC Code of Conduct at all times acting as a professional role model leading by example in regard to behaviours, attitude, conduct and appearance To demonstrate an understanding of and adhere to Trust Policies and procedures relating to Safeguarding of children Document and escalate concerns ensuring the safety of the young person at all times Support families and young people in times of distress and anxiety and escalate to transition co-ordinator, consultants or other relevant healthcare professional Contribute to My Performance Appraisal (MPA) with manager or supervisor Contribute at meetings/groups to represent & promote the needs of young people & the transition service To support all health care staff on matters relating to transitional care promoting the development of clinically effective care that is seamless between settings. Support staff and engage with various specialities to deliver a joint approach to transition Contribute to development and adherence to transition audit processes Highlight barriers during transition process so as to contribute to the constant evaluation & improvement of the transition service The post holder will work across the whole trust, organisation, boundaries and locations The post holder will be responsible for maintaining key links with the local transition network To maintain high standards of Transitional Care the post holder will help to identify gaps in current service provision and assist in developing effective, efficient patient pathways If clinical need/acuity demands, to support the clinical area with any acute nursing tasks where they fall within the limits of your capability To support and work with the transition co-ordinator in delivering transition training to all members of staff. Identify own education and development needs, access appropriate learning opportunities to ensure clinical knowledge is continuously updated Participate in clinical supervision / preceptorship / mentorship as required To assess and evaluate the health education needs of the young person and their carers about their condition including medication and treatment and to provide verbal and written information about the conditions that is age and culturally Maintain own clinical development needs to enable working on the childrens wards in times of need / high demand For further information see job description and personal specification attached
