We are seeking a confident, experienced, enthusiastic, and motivated Service Coordinator who is passionate about improving patient care to join our Physical Health and Wellbeing Service. This role is essential in helping us deliver our Bradford District and Craven action plan 'Improving Physical Health and Reducing Premature Mortality for People with Severe Mental Illness, Learning Disabilities and Autism Spectrum Condition'.
As a Service Coordinator, you will support the team with service delivery and lead on service-wide coordination, including performance reporting and system analysis. Your responsibilities will include:
1. Supporting service improvements, administration, and site coordination.
2. Ensuring compliance with health and safety and service quality standards.
3. Working toward trust targets, values, strategic objectives, and business planning.
