Are you passionate about making a difference in the lives of children with disabilities and their families? Stockport Council is looking for a CWD Short Breaks Coordinator to join our dedicated team.
About the Role: As a CWD Short Breaks Coordinator, you will play a crucial role in assessing the eligibility and needs of children requiring short breaks, securing appropriate provisions, and ensuring these services effectively support the child and their family. You will work closely with families, social care, education, healthcare professionals, and community partners to develop, implement, and review tailored short breaks.
Key Responsibilities:
Assess eligibility and need for short breaks and coordinate appropriate provisions.
Work collaboratively with families and partner agencies to ensure support meets the needs of children with disabilities.
Provide specialist knowledge and guidance on short breaks to colleagues and other agencies.
Maintain accurate records, prepare reports, and present to the CWD short breaks resource panel.
Attend and contribute to multi-agency meetings to support children and families effectively.
Ensure the short break plan aligns with other support plans, including EHCPs and early help interventions.
Promote inclusion and advocate for children with SEND and their families.
Adhere to Stockport’s safeguarding procedures and work within relevant legislative and policy frameworks.
About You: We are looking for an individual with:
Experience working with children with Special Educational Needs and/or Disabilities (SEND).
Knowledge of Stockport’s SEND offer, policies, and procedures.
Strong assessment, negotiation, and advocacy skills.
Experience in multi-agency working and case management.
A commitment to inclusive practices and a strengths-based approach.
The ability to build positive relationships with children, families, and professionals.
Excellent communication, planning, and organisational skills.
A valid UK driving license or the ability to travel between locations as needed.
