Primary Care Advanced Pharmacist / Team Co-ordinator

Location: Pharmacy Team, Aberdeen City HSCP
We are looking for a motivated and enthusiastic Advanced Pharmacist to join the Aberdeen City H&SCP Pharmacy Team. Applicants should have experience of working in an Advanced Pharmacist Clinical post and now wish to further develop their leadership, management, planning and organisational skills.
This full time post will support the Aberdeen City Lead Pharmacists with the management/operational needs of delivering pharmacotherapy and prescribing support services across the HSCP, working alongside our other team co-ordinator. The post holder will have regular clinical input with an approximately even split across the week of clinical time and operational support
You will have a role in co-ordinating, supervising, training and mentoring other members of the pharmacy team with additional delegated responsibility for leading on specific areas of work on behalf of the Lead Pharmacists. This will include working in / supporting the operational running of our pharmacy hub.
As an Advanced Pharmacist within the city wide team, you will also be expected to lead, oversee and develop the following within your allocated GP practices: medication / polypharmacy reviews; medicines reconciliation; actioning of hospital discharge letters, actioning of acute and repeat prescription requests; providing prescribing advice to any member of the health care team; reviewing prescribing data; undertaking audit; answering ad-hoc queries; attending multi-disciplinary team meetings; and supporting social care colleagues with medication-related issues in care at home services.
Candidates must have a degree in Pharmacy and be on the practising register of the General Pharmaceutical Council (GPhC). Candidates must also be an Independent Prescriber (IP) and be willing to develop in this role. Completion of a clinical diploma/NES Advanced Competency Framework (or equivalent knowledge demonstrable through experience) is also essential. Candidates should have good IT skills; excellent communication and interpersonal skills; be a car owner and driver; and be able to work in any location in Aberdeen city.
