What You Will Do:
1. Patient Assessment and Support: Conduct comprehensive assessments for patients at the front door of the Emergency Department, ensuring the effective planning of patient pathways and discharge processes.
2. Equipment Assessments: Evaluate and ensure that the necessary equipment provisions are in place for patient care, promoting their comfort and safety.
3. Multidisciplinary Collaboration: Work closely with healthcare professionals across the Emergency Department to facilitate efficient and rapid response services, promoting admission avoidance and discharge planning.
4. Documentation and Communication: Maintain accurate and detailed patient records, ensuring clear communication with the team to optimise patient outcomes.
What We Are Looking For:
1. HCPC Registration: You must be a registered Occupational Therapist or Physiotherapist with the Health and Care Professions Council (HCPC).
2. Experience: At least two years' experience in front door admission avoidance, rapid response areas, or discharge planning.
3. Driving Licence (Advantageous): While not essential, having a valid driving licence and the ability to travel between sites would be beneficial.
4. Effective Communicator: Strong communication skills to engage with patients, families, and multidisciplinary teams to ensure high-quality patient care.
