The successful candidates will work within theReadinglocality health visitingteam butwill need to be flexible around working fromdifferent sites as they support children aged 0-5, their families and Health Visiting colleagues.They will support the implementation of the Healthy ChildProgrammeunderpinned by the Solihull Approach model of working.
The ideal candidates for these roles willdemonstratean interest and passionfor health visiting.They will need to be a good team player withgood communicationand interpersonal skillsandwith the ability to work effectively with service users and a range of other agencies and professionals.
The Specialist Community Public Health Nursing Programmeis offered by Berkshire Healthcare Foundation Trust at either Oxford Brookes or Bucks New University.
• To provide support to families within a variety of community settings under the direction of the Health Visitor,taking into accountthe diverse social, health and wellbeing needs of the community.
• To plan and deliver health enhancing activities for children and their families to include working ingroups, childhealth clinics, developmental review clinics, and on an individual basis.
• To provide support to families using the Solihull approach model in behaviour management, parentingskillsand delivering programmes of care to vulnerable families asidentifiedby the health visitor.
• To support breastfeeding and parent infant relationships applying the UNICEF UK Baby Friendly Initiative standards.
• To liaise with other agencies and services to provide integrated and cohesive support for children and their families, under the direction of the health visitor.
• Work with the HV team to achieve the Key Performance Indicators (KPI) especially focussing on the antenatal and the6-8 weekpostnatal contacts
