Requirements:
4. Care Coordination : Ensuring all aspects of a patient’s care are organized and communicated across providers.
5. Health Assessments : Regular evaluations of the patient’s health status.
6. Personalized Care Plans : Tailored plans to meet the specific needs of each patient.
7. Support Services : Access to social services, mental health support, and community resources.
What we are looking for in a Social Worker:
8. Social Work England registration
9. Eligible to work in the UK
10. Hold a full UK licence
11. Post-qualified experience
12. Degree level or equivalent in Social Work
