Are you passionate about providing exceptional care to elderly patients? Join our dedicated team on Medical 2, our care of the elderly ward at the Princess Royal University Hospital site of King's College Hospital NHS Foundation Trust.
As a Band 5 Staff Nurse, you'll be part of a ward that takes pride in being dementia friendly while delivering comprehensive care to our elderly patients. You'll work within a supportive multidisciplinary team that champions person-centred care and dignity for all patients.
Responsibilities:
* Deliver specialised, compassionate care to elderly patients, with particular emphasis on supporting those living with frailty and dementia.
* Champion our dementia-friendly initiatives and ensure best practice in person-centred care.
* Work collaboratively within our multidisciplinary team to create and maintain a therapeutic environment.
* Assess, plan, implement and evaluate individual patient care needs, considering both physical and cognitive aspects.
* Support families and carers, providing education and guidance throughout their loved one's hospital journey.
* Participate in quality improvement initiatives to enhance our services.
* Monitor patients' conditions closely and report changes to the multidisciplinary team.
* Administer medications and treatments as prescribed, maintaining accurate documentation.
* Participate actively in discharge planning to ensure safe transitions of care.
