This is an exciting, permanent role, in the Tiverton and Cullompton Urgent Community Response Team, for an enthusiastic and motivated Specialist Nurse.
The UCR team provides a 2 hour response to admission avoidance patients within the community, and supports with discharge assessments from the acute hospital back home.

Job responsibilities
This role is based within the Urgent Community Response team in Tiverton. The post holder will be part of an integrated health and social care team working to provide high quality, innovative care to patients in their own homes. The collaborative aim being, maintaining patients in their preferred place of care wherever possible enabling them to maximise their independence and optimise their quality of life.
The post holder will be responsible for carrying out holistic assessments and a wide range of clinical interventions including end of life care, wound care management, continence care, medication management and long-term condition management. Transferable clinical skills will be supported with high quality training, and there is opportunity for ongoing personal and professional development, supported by regular supervision and annual appraisals.
The post holder will also be responsible for the triage, planning, implementation and evaluation of personalised patient centred care, in partnership with our patients and those important to them. Therefore, excellent communication and IT skills, with the ability to prioritise, problem solve and be a team player who is adaptable to change are paramount.
As a Band 6 the post holder will support with supervision and the development for Band 5 and unregistered members of staff.
Working Pattern: 37.5 hours per week. The service covers 8AM - 8PM across 7 days. The post holder would be required to work on a rota basis within these hours. Weekends are currently 1 in 4 on average. There is opportunity to work long days. Working pattern to be discussed at recruitment.

Pay - The salary advertised for this position reflects the Agenda for Change 2024-5 rates, inclusive of a 5.5% pay award implemented from October 2024. If you join before this date, the pay award will be backdated to April 2024 or your start date, whichever is later. Until then, remuneration will be based on the 2023-4 pay bandings.
