Primary Purpose of Role To work with START colleagues and other professionals to provide a short-termassessment and reablement service for a maximum of six weeks to assessed service users in their own homes and within the community. Responsibilities include; All tasks undertaken need to take into account the individual needs of each customer and their particular abilities together with the specified outcomes contained in the service users care plan To encourage a service users independence and activity appropriate to their abilities in all aspects of their day-to-day living. To assist a service user in their personal care for example: washing/toileting/dressing/undressing. To provide assistance with other tasks for example: cleaning/laundry/shopping/collection of pension. To assist a service user in the preparation of meals/drinks/snacks helping them to regain the skills required for such tasks as appropriate. To assist a service user with their medication in strict accordance with laid down procedures. To assist a service user to transfer using appropriate moving techniques and/or aids and equipment. To undertake regular training as required by the START team to ensure the working practices are consistent and up-to-date with all current thinking regarding service delivery, with particular emphasis on reablement. To contribute to the supervision of particularly vulnerable service users and to be aware of the appropriate action to be taken in an emergency. To monitor and observe a service user for possible signs of abuse and report accordingly. To work with Physiotherapists/Occupational Therapists and other professionals to assist a service user in regaining skills required for independent living.
