The rehabilitation service works towards individuals' recovery by using the recovery model and rehabilitative model of care and support with adults with severe and enduring mental illness.
Main duties of the job
The purpose of the role is to provide day-to-day (shift) nursing leadership within a rehabilitation inpatient service to support individuals to meet their agreed goals for rehabilitation and secure ongoing placement so that we treat people in the least restrictive environment.
The rehabilitation practitioner will provide specialist nursing advice to inform assessment (including risk assessment), care planning, treatment, and review.
They will lead the shift and ensure meaningful activity aimed at rehabilitation and improving life skills is provided daily. They will ensure that the group programme is adhered to and that all staff are meaningfully engaged with our service users throughout their shift.
Through advanced clinical nursing and leadership skills, they will ensure that the unit provides a safe and secure environment in which rehabilitation is the focus. This will ensure comprehensive risk management plans are in place that foster positive risk taking to enable people to recover.

Job responsibilities
Carry out duties as required on a daily basis, to include the following:
1. Ensure a high standard of nursing input in the application of clinical procedures, including assessment, risk assessment, care planning, implementation and evaluation, medication management, physical investigations, and implementation of the Recovery Approach.
2. To liaise with the ward manager in ensuring the safe and effective delivery of care, in line with national, local, and Navigo policy, through supervision of junior staff and audit processes.
3. To undertake incident/accident reports, safeguarding, complaints, investigations to ensure the safe running of the ward.
4. Attendance at weekly M.D.T meetings and effective presentation of clinical reports and information.
5. Liaison with other M.D.T disciplines to ensure effective team working and consistent approaches to care delivery.
6. Oversee the effective delivery of the MDT process.
7. Oversee the correct implementation of negotiated care plans.
8. Supervise the completion of nursing reports by Key/Associate Nurses (C.P.A/S117, M.H.R.T, Hospital Managers, and MDT) in keeping with specified and statutory timescales.
9. Attendance at six monthly patient C.P.A/S117 meetings.
10. Attendance at M.H.R.T and/or Hospital Managers Hearings in the absence of the Key/Associate Nurse.
11. Ensure effective management of medication through training delivery, and carry out audits to ensure adherence to M.H.C. policy and N.M.C. guidelines.
12. Oversee Key Nurse liaison with external Care Co-ordinators and Commissioners.
13. Dissemination of action points from Clinical Governance meetings.
14. Support the ward manager in conducting and addressing issues of complaints and investigations.
15. Liaise with the ward manager in addressing issues involving whistle blowing.
16. Liaise with Key/Associate Nurse and ward manager in establishing and maintaining therapeutic relationships with service users' families (where appropriate).
17. Ensure adequate staffing to cover unit requirements and liaise with ward manager to address any deficits, taking into consideration financial budgets.
18. Carry out assessment visits as part of the M.D.T, assessing potential patients and preparing assessment and treatment reports.
19. Monitor, undertake and oversee ward-based risk assessments and management plans.
20. Carry out and monitor annual appraisals of nursing staff and support workers, as directed by ward manager and ensure regular evaluation of PDRs.
21. Have direct line management for staff nurses, nursing associates, and nursing assistants.
22. Undertake, monitor and ensure monthly supervision of nursing staff.
23. Chair monthly ward staff meetings, ensuring delegation and implementation of action points.
24. Provide Mentorship/Preceptorship to newly qualified or newly appointed nursing staff (including nursing students, where applicable).
25. Liaise with ward manager in the identification of external and in-house training (including mandatory training) for specified staff.
26. Provide temporary cover for ward manager in the event of their absence, as required by the Hospital Manager.
27. Undertake interviewing of new ward staff, at nursing and support level.
28. Attendance at the morning staffing meeting in the absence of the ward manager.
29. Support the ward manager in all personnel-related issues.
30. Oversee and coordinate monthly audit of all office files and develop and implement action plans to address identified deficits.
31. Co-ordinate monthly clinical audits and implement action points to address any identified deficits.
32. Monthly input of KPI data, including senior nurse report, training, supervisions, appraisals, revalidation, and sickness monitoring.
33. Ensure service user finances are managed according to M.H.C policy and assist with audit as required.
34. In conjunction with the Clinical Administrator, monitor and ensure that legal processes and documentation are in line with MHA (1983) guidelines and M.H.C policy and procedures.
35. Support the Associate Director of Nursing and Quality in the completion of contractual and commissioner-related documentation.
36. Oversee the completion of individual weekly therapeutic timetable.
37. Monitor the implementation of ward structured day, including the specified hours of meaningful activity.
38. Review the completion of monthly audit of Recovery focused 1:1 Key Worker and Key Nurse discussions.
39. Monitor three-monthly review of Recovery Star by Key/Associate Nurse or Key Support Worker. Monitor monthly review of HoNoS. Monitor monthly patient file audits and oversee actions required and monitor that they have been completed.
40. To drive forward own development plan through supervision and appraisal.
41. Other duties as required within scope of experience, training, and competence.

Experience
* 18 months experience working within a relevant mental health service with service users with acute or severe and enduring mental health needs.
* Experience of working with service users with complex and challenging needs.
Knowledge
* Evidenced knowledge of relapse prevention and recovery models.
* Advanced IT skills (Microsoft office applications).
* Psychosocial interventions.
* Use of clinical assessment measures.
Qualifications
* Current UK RNMH/RMN or HCPC professional registration (provide PIN or registration number to evidence).
* 032/33 or ENB998.
* Clinical Supervision Training.
