There is 0.57 WTE position (21.5 hours per week) available.
Hybrid working available, no weekends or bank holiday working and no on call.

We have some very exciting opportunities to join our exciting Community Frailty Virtual Ward project. This project forms part of a wider plan to improve urgent and emergency healthcare delivery, across Plymouth, the South Hams, and West Devon.

Our aim is to provide high-quality, patient-centred care in the comfort of their own homes, reducing hospital admissions and length of stay in hospital and enhancing overall well-being through enhanced care coordination.

Main duties of the job
The Role
As a Clinical Pharmacy Technician, you will be a cornerstone of healthcare delivery and will play a critical role in our new multidisciplinary team, covering with the CFVW Pharmacists the six new frailty virtual wards that we have planned for our locality.
Here Is What You Will Do
* Medicine reconciliation.
* Home visits and medicines optimisation.
* Accurate record keeping regarding medications.
* Working closely with the Community Frailty Virtual Ward Pharmacist.
* Answering medication queries raised by the multidisciplinary team and providing support to the Community Frailty Virtual Ward Team.
* Discharge facilitation.
* Work within integrated care across several multi-disciplinary teams including, Mental Health, District Nursing Discharge teams, Acute GPs, and Social Workers.

Together, you will drive seamless transitions of care and ensure outstanding outcomes for our patients.
In this role, you will help coordinate clinical pharmacy services across the Virtual Wards, employing a holistic approach to medicines optimisation across a variety of healthcare settings. Beyond daily responsibilities, you will be at the forefront of innovationclinical audits, spearheading quality improvement projects, and contributing to innovative research that pushes the boundaries of whats possible in-patient care.

You will also play a key role in shaping the future of Medicines Optimisation within Pharmacy Services, providing strategic input while maintaining a strong presence within the organisation. Your passion for excellence in pharmacy will help develop and inspire the Virtual Ward Pharmacy Team, as well as healthcare professionals including Non-Medical Prescribers and other Allied Health Care Professionals, ensuring the highest standards in medicines governance and clinical care.
