To complete the ongoing assessments for young people in crisis on the ward, or routine assessments in the community.
1. To complete all assessment paperwork, contact any agencies, and make appropriate referrals.
2. To complete risk assessments as appropriate and develop safety and care plans.
3. To collaborate with other partner agencies, e.g., children's services/safeguarding, etc.
4. To provide ongoing support work for young people in crisis, refer as appropriate, and review upon return to services.
5. To conduct sessional work with young people on an ongoing basis as directed.
6. To manage risk, complexity, and priorities.
7. To manage a caseload and escalate any matters of concern or uncertainty.
8. To work in a team, supporting and being supported by colleagues.
Full-time position, Monday to Friday, 9 AM to 5 PM, with some flexibility in hours. Occasional additional days may be required.
