Job summary Shirehampton Group Practice has an exciting opportunity for a practice nurse to join our treatment room team We are looking for a motivated, organised nurse to work full time at our friendly practice. Our ideal candidate will work Monday and Fridays and flexibility of compressed days is on offer. Our ideal candidate will have additional long term condition qualifications or a willingness to learn. Main duties of the job To assess, plan, implement and monitor care for patients and to promote health and wellbeing within the practice and PCN population. You will work unsupervised to carry out assigned tasks involving direct and indirect care in support of the treatment room nursing team, and within your remit and competencies. To promote and maintain high standards of nursing care in accordance with the NMC Code of professional standards of practice and behaviour for nurses. To provide Chronic Disease Management clinics to the level of competency achieved within the confines of Practice Protocols and Procedures Assist in the monitoring and observation of patients. Ensure that the principles of Infection Control are always adhered to. Report any concerns relating to a patients condition to either a senior nurse or, in their absence, the doctor on-call. On occasions when a more senior nurse is not on duty or available to the practice nurse, the on-call GP must be approached with any matters of concern. Advise patients on general health care and minor ailments, plus giving opportunistic health promotion advice as appropriate. Attend and participate in relevant staff meetings and communicate effectively any relevant information within the team. Use information technology for updating patient records and retrieving audit information as required. To fully participate in administrative and professional responsibilities of the Nursing Team. To fully participate in the education and training of students in all disciplines as required. To liaise with other Health/Social Professionals as appropriate.
