Are you passionate about patient care and ensuring smooth transitions from hospital to home? Mansfield Community Hospital is seeking a dedicated Discharge Coordinator to join our dynamic team. This role is crucial in the discharge process, ensuring all necessary arrangements are in place for the safe discharge of our patients.
The successful candidate will support the planning and focus on patients with a predicted date of medically safe discharge within 48 hours and support their discharge process. You will develop close working relationships with all members of the Multi-Disciplinary Team (MDT), both internal and external to the Trust, to support and coordinate the discharge process.
This role is part of the wider corporate capacity, flow, and resilience departments, reporting directly to the Chief Operating Officer. The discharge service works closely with community colleagues on the Discharge to Assess model of care and supports the management of patients with long lengths of stay, specialist discharges (End of Life and Fast Track), and complex discharges. The service may also overlap with other department functions for bed flow and virtual hospital management.
