Job summary This post is positioned within the School Health Service, part of the Children's Community Services Directorate in the Manchester Local Care Organisation. They operate citywide across Manchester working in partnership with schools to improve the health outcomes of children, young people and their families. The Healthy Weight Team aims to reduce the levels of severe obesity within nursery and reception aged children, by providing effective and comprehensive weight management support to the parents/carers of children identified as severely obese through the National Child Measurement Programme (NCMP). The Healthy Weight Team work with families across Manchester on an individual basis to provide dietary, physical activity and behaviour change advice and support, either in the school or home setting. The post holder will work as a member of the Healthy Weight Team delivering interventions and packages of care delegated to them by a registered nurse or dietitian. The post holder will work in partnership with families in the home, school and local health centres. The post holder will deliver agreed health information and support to children and families working across the community, universal, universal plus and universal partnership plus spectrum of the Healthy Child Programme (HCP). Main duties of the job Undertake home and school visits, and telephone consultations to families to implement programmes of weight management support under the direction of the Healthy Weight Nurse To signpost parents and children to appropriate services/agencies for additional support as required To discuss and agree short, medium or long-term goals, prioritise care and develop and implement appropriate plans of care to support families around weight management. To work without direct supervision in the implementation of care plans to meet prior agreed goals, outcomes and evaluate the effectiveness of interventions and make any necessary modifications. To provide advice to families and professionals around weight management and healthy lifestyles. To maintain comprehensive and accurate, documentation and data systems in line with legal and trust requirements. Undertake the measurement and evaluation of your work and current practices through the use of outcome measurers and support the application of practice based on best available evidence.
