Clinical Reviewer - Learning Disability Nurse

Main duties of the job
As a Clinical Reviewer your main duties will include:
Clinical Expert and Chair of Care and Treatment Reviews (CTRs)
* Writing the KLOE report for CTRs.
* Ensuring the persons views are heard during a care and treatment review as well as assessing the clinical needs of patients and people within the community who have a learning disability and/or autism.
* Leading conversations within multi-disciplinary teams during CTRs.
* In discussion with the CTR panel devise SMART objectives for the person and their team.
LeDeR reviews
* Using your professional curiosity to find information about a person's life and death.
* Analysing information against standards for good care and treatment.
* Writing reports highlighting effective practice and areas for improvement.
* Communicating with other professionals, families and providers to gather information.
* Presenting reports to local ICB quality assurance panels.
* Building a relationship with the person with ASD and/or LD and gathering information to write a life plan.
* Liaising with the person's circle of support to gather clinical and social needs.
* Analysing information in order to develop recommendations.
* Presenting information in a written format which is professional yet person-centred.
* Talking to families about the person's history and needs.
* Providing reports and updates on a regular basis to outside agencies and your line manager.

Person Specification
Experience
* Previously used the LeDeR platform.
* Previously used KLOE spreadsheet for CTRs.
* Experience working within service level agreements, e.g., KPIs and contracts.
Qualifications
* Registered Clinician, e.g., Doctor (Psychiatrist), Psychologist, Nurse, Occupational Therapist, Speech and Language Therapist, Behaviour Analyst, in good standing with the relevant body.
* Experience of working within transforming care, learning disability, mental health or neurodiverse services.
* Non-Medical Prescriber.
* Level 3 in safeguarding adults.
* Experience of working within adult secure services.
* Evidence of relevant CPD.
* Experience as chairing/clinical expert for CTR/CETRs.
* Experience as reviewer for LeDeRs and ILPs.
