An opportunity has arisen for a Band 8A Macmillan Clinical Nurse Specialist Palliative Care Lead. The suitable candidate will also get an opportunity to develop their wider skills in palliative care through clinical leadership, clinical assessment of palliative care or end of life patients and support the team in developing and delivering the service.
The post is that of a specialist practitioner with palliative care and End of Life Care experience and has the ability and skills of caring for patients with an advanced life-threatening illness. The postholder will be able to make informed decisions within this specialty and scope of professional practice, involving patients and the public to ensure agreed standards, key performance indicators and objectives are met.
The post holder is an experienced nurse with a palliative care qualification, who will act as a clinical leader within Palliative and End of Life Care across the Trust. You will be able to autonomously assess any palliative care or end of life patients including history taking, clinical assessment, diagnosis, treatment and evaluation of care.
You will be expected to work collaboratively with the Palliative and End of Life team to meet the needs of patients and will lead and support the team in developing and delivering the service ensuring adherence to policy and procedures.
Main duties of the job
This post will work closely with community teams in Enfield, Haringey and with hospices. The team works very closely with the community teams in Enfield, Haringey and with hospices including North London Hospice, St Joseph's Hospice and Isobel Hospice in Hertfordshire. North Middlesex University Hospital (NMUH) is also part of the Haringey Community Partnership Group.
The Palliative Care team currently consists of 1 WTE Macmillan Palliative Care Team Lead, 4 WTE band 7 Clinical Nurse Specialists, 1 WTE band 6 Developmental Associate Clinical Nurse Specialist, and 1 WTE Fast Track Discharge Co-Ordinator. There is also 1 WTE band 8B Nurse consultant for Palliative Care and End of Life.
The team are supported by 2.6 WTE Palliative Care Consultants, a trainee GP, 1 IMT3, 1 Specialist registrar. Currently the palliative care team provide a 7-day palliative care service 9-5pm including Bank Holidays.
The post will play a key role working in all areas of the Trust where Palliative and End of Life patients may be situated. This includes the acute medical unit (AMU/AAU), Emergency department, frailty wards, Critical care unit and the community stepdown wards at Chase Farm.
The post holder will provide transformational leadership to staff at all levels, providing a highly visible presence and ensuring the provision of a high quality, responsive service with the patient at the center of care delivery inclusive of family and significant others where appropriate.

Professional
* To promote personal, professional & clinical expertise, through the regular evaluation and updating of palliative care and End of Life nursing practices.
* To identify areas of risk and poor practice and address, through appropriate clinical governance structures.
* To comply with the Nursing & Midwifery Council (NMC) Code of Professional Conduct and statutory rules governing professional nursing practice and all trust guidelines.
* To develop and maintain a professional portfolio and maintain clinical study requirements in compliance with NMC recommendations and provide evidence of revalidation.
* To critically reflect on one’s own performance, for example, through the use of clinical supervision and the Trust’s appraisal system.
* In conjunction with the North Middlesex University Trust & North London Cancer Network to work towards the recommendations of the National Institute for Clinical Excellence Improving Supportive and Palliative Care for Adults with Cancer within the Trust.
* To participate in working groups related to Palliative Care & End of Life and presenting at local and national levels.
Expert Clinical Practitioner
* To receive direct referrals and manage own caseload autonomously, without supervision. Taking a lead on more complex cases and working with the Multidisciplinary team.
* Ensure that people affected by cancer are aware that they are interacting with a Macmillan Professional and know about the full range of resources and services available through Macmillan.
* To act as patients advocate and be present at Best interest meetings.
* To maintain the highest standards of care through assessment, planning, implementation & evaluation of the physical, psycho-social, spiritual and cultural components of all patients and their carers.
* To use specialist knowledge of symptom control to inform medical teams of the appropriate prescribing of medication to control symptoms. As a non-medical prescriber, prescribe medications for symptom control within your scope of practice.
* To communicate information and advice given, clearly on EPR systems and/or verbally for Multi-Disciplinary Team Colleagues.
* To empower the staff within wards and clinical areas who are completing the delivery of care to palliative or End of Life Care patients by working with the multidisciplinary team.

Person Specification
Education and qualifications
* Educated to degree level & RN
* Evidence of recent professional development
* Palliative Care qualification and/or extensive experience in the speciality.
* Educated to Masters level
* Non-Medical Nurse Prescribing Qualification.
