If so, we have an exciting 3-month opportunity for a suitably qualified/experienced Community Nurse to apply for the demanding role of Integrated Community Team Lead.
Applicants must be a Registered Nurse in possession of their Specialist Practice Award in District Nursing (postgraduate) qualification. Additionally, candidates must either hold or be willing to pursue a relevant Masters qualification.
The successful applicant will manage and lead a team of community nurses, ensuring the seamless and safe delivery of 7 day a week community nursing care to a large caseload of acute and chronically ill patients. There is a minimum requirement for a highly flexible and enthusiastic approach to this challenging role as well as an ability to travel between sites in a timely manner.
The interim post is based with the North Coastal Integrated Community Team based in Fishguard Health Centre, although the post-holder may be required to work across both North and South Pembrokeshire Localities to meet the needs of the Service.
Main duties of the job
Accountable for management of nursing caseload to include leading/developing core nursing establishment in delivering compassionate, high quality, patient-centred nursing care, providing fair, honest and measured people management.
Provide professional nursing leadership with support/guidance (in line with NMC standards).
Provide expert clinical advice/intervention on specialist complex issues within own field and undertake management/monitoring activities in line with legal/professional requirements, statutory rules and Health Board policies relating to evidence-based practice.
Responsible for efficient management of allocated budget effectively deploying the nursing team, utilising allocated resources to timely raise financial expenditure concerns to appropriate personnel.
Ensure complex needs of patients with multiple pathologies and families are addressed in a sensitive/empathic manner including supporting the assessment/implementation/re-evaluation of continuing health care packages of care.
Work/communicate effectively as a member of senior clinical/nursing leadership team, collaborating with other Team Leads and Clinical Lead Nurse, including deputising for Clinical Lead Nurse when required.
